Job Summary:
Under the supervision of the Program Director, works with the clinical staff in the assessment of client’s needs. Resolves all client issues by escalating concerns to Clinical or Administrative team. Client retention is the primary focus, and all efforts to ensure client has positive treatment experience is top priority. Assists management teams in meeting clients’ mental and physical health needs through communication and available resources to ensure the clients’ ongoing well-being. Provides necessary case management to all clients in treatment to guarantee that their current needs are met, and their discharge planning is in place for a smooth transition to aftercare services.

Essential Functions and Responsibilities:
  • Develop individualized care plans in collaboration with patients and treatment teams, incorporating after-care planning from the onset of admissions.
  • Advocate for patients’ rights and needs within the healthcare system.
  • Assist clients in completing surveys at four stages of treatment: admission, in treatment, discharge, and post-discharge.
  • Maintain accurate and up-to-date case notes and documentation of services provided.
  • Facilitate communication among clients, psychiatrists, and other relevant parties.
  • Coordinate and schedule third-party appointments for clients, verifying insurance coverage, and refer them to specialized services like substance abuse treatment and vocational training (TMS and Ketamine).
  • Maintain communication with external facilities, such as substance abuse or eating disorder treatment centers, to ensure seamless transitions and the client’s return to our mental health facility as needed.
  • Work closely with Utilization Review regarding any insurance requirements.
  • Conduct weekly case management sessions with each client and document them in the EMR on the same day.
  • Handle all discharge planning and reach out to aftercare facilities or therapists to schedule the first aftercare appointment alongside the client. 
  • Complete the discharge case management note within one week following the client's discharge.
  • Provide culturally sensitive care and support, respecting clients' diverse backgrounds and experiences.
  • Actively listen to clients’ concerns, resolve issues when possible, and report them to the appropriate manager when necessary.
  • Encourage clients to make the most of their treatment experience by emphasizing the importance of rehabilitation and recovery.
  • Coordinate and facilitate support groups or discharge planning groups as per the curriculum for clients and their families to address specific mental health issues or coping skills. 
  • Participate in treatment team meetings and case conferences to discuss clients' progress and coordinate care plans with other staff members.
  • Assist clients in completing necessary paperwork for FMLA, EDD/SDI, COBRA, or any other documentation needed for ongoing treatment or work.
  • Establish and maintain relationships with community organizations and service providers.
  • Identify and connect clients with community resources for housing, food, clothing, and other essential needs.
  • Attend training sessions and professional development opportunities to enhance skills and knowledge.
  • Carry out other duties/responsibilities as delegated.

  • Excellent organizational and time management abilities to manage multiple cases effectively.
  • Understanding of mental health disorders and treatment options to provide informed support.
  • Familiarity with community resources and support services available for clients.
  • Strong analytical and problem-solving skills to assess situations and develop effective solutions.
  • Basic computer skills, including word processing, spreadsheets, calendars, and presentations.
  • Familiarity with EMR systems, preferably KIPU, but not required.
  • Strong customer service relations and performance improvement skills.

Education and Experience:
  • Has a working knowledge of psychopathology, diagnostic assessments, addiction, and related interventions.
  • Demonstrates an understanding of addiction in psychiatric illness.
  • Three to five years of working in a mental health treatment center environment.
  • Bachelor’s degree preferred.

Physical Requirements:
  • Frequent sitting must be able to remain in a stationary position at least 50% of the time.
  • Must be able to lift and carry light objects and parcels as an incidental part of their job.
  • The position needs to occasionally move about inside and outside the facility frequently.
  • Constantly operates a computer and other office equipment.
  • Frequently communicates with other employees; must be able to hear and speak English clearly.
  • Ability to drive in between facilities is required.
